What information does the intake questionnaire collect?

The Posterity intake questionnaire is designed to capture all the information your attorney needs to begin drafting your estate planning documents. Here's a complete overview of each section.

Personal Information

Full legal name, date of birth, Social Security Number (optional), current address, contact information, and citizenship status.

Tip

You can skip the SSN field — your attorney can collect this separately in person.

Family Structure

Marital status, spouse information, children (biological, adopted, step-children), grandchildren, and any dependents with special needs. This section also captures information about prior marriages.

Assets & Property

Real estate (primary residence, investment properties), financial accounts (checking, savings, investment, retirement), life insurance policies, business interests, and personal property of significant value.

Tip

You don't need exact values — estimates are fine for the intake process.

Wishes & Preferences

How you'd like assets distributed, who you'd name as executor, trustee, and guardian for minor children, and your healthcare preferences including power of attorney designations.

Review & Submit

Before submitting, you'll have the opportunity to review all your answers, make any corrections, and add notes for your attorney. Your submission is encrypted and delivered directly to your attorney's dashboard.
